In South Africa, the Ananzi search engine ranks pages almost exclusively based on the meta-tags, and even Google and Yahoo! are beginning to take into account the keywords meta-tag, with one proviso; the keywords must accurately reflect the content of the page.
The search engines use this tag to identify which of the keywords in the content you feel are the most important, so that they can return the most accurate results possible.
